The term ‘FD Sports Massage’ or ‘I’, ‘my’ or 'me' refers to the owner of this website whose registered address is 7 Chinery Close, Chelmsford, Essex, CM1 7QY. The term ‘you’ refers to the viewer or user of my website or services.
I may collect and process data about you (i) when you send an enquiry (ii) when you book an appointment (iii) during an appointment or iv) if you subscribe to my mailing list. This data is used in the following ways:
i) any personal details provided by you when sending an enquiry, whether via the online enquiry form or by any other means, will be used solely for the purposes of responding to that enquiry.
ii) any personal details provided by you at the time of booking an appointment will be used for appointment confirmation and reminder purposes only. These will generally be sent by email but in the case of last minute appointment changes I may contact you by telephone.
iii) for the purposes of providing treatment you will be asked to provide information on your current and past medical history so that I can ensure treatment is safe and appropriate for you. I will also make notes which may include details concerning your medication, treatment and other issues affecting your health. This data is always held securely and must be kept for 8 years for insurance purposes. There may be circumstances related to your treatment and on-going care that will require the sharing of your medical records with other healthcare practitioners e.g. GPs, consultants, surgeons and/or insurance companies. Where this is required I will always inform you first unless we are under a legal obligation to comply.
iv) if you subscribe using the online form provided on this site, or via the Patient Consultation form you complete at your first appointment, you are consenting to receive emails from time to time with news and offers. You can choose to unsubscribe from these at any time should you wish to do so using the link provided on the emails sent to you or by clicking here.
I use strict procedures and security features to try to prevent any unauthorised access of your information. However, the transmission of information via the internet is not completely secure. Although I will do my best to protect your personal data, I cannot guarantee the security of any data transmitted on my site. As such, any transmission is at your own risk.
I will never sell your details to anyone else, or pass them to a third party unless required to do so by law or specifically requested to do so by you.
